What is Support Coordination?

Support coordination is a case management service through the Division of Developmental Disabilities (DDD) in NJ. Much like a school case manager, a support coordinator helps individuals and families set up services, and manages their annual budget through the division. In addition, support coordinators act as a liaison between DDD, service providers, and individuals to provide information and ensure that an individual is safe and able to transition seamlessly into adulthood and reach their goals.
